Online Patient Forms

Upon entering our office our Wellness Team will greet you and welcome you as a member of our family.  We will request that you complete our patient forms online prior to your first visit.  This provides us with critical information about yourself and your condition, so that our award-winning Doctors can better serve you.


Next you will have a consultation with the Doctor to discuss your health-related problems, concerns, health goals as well as potential treatment options. Your first visit is designed for the Doctor to learn more about you, your condition and your expectations to determine whether Corrective Chiropractic care will help you obtain your true potential.

TPC Biostructural Examination

After your consultation, the Doctor will perform a complete chiropractic examination to determine how well your nervous system and "lifeline" (spinal cord) is functioning (Please note, nothing will be done in our office without your consent).

Our standard tests are safe and pain-free and include:

1. Palpation evaluation

2. Chiro-specific testing including reflexes (DTR), ability to turn and bend (ROM), and other specific neurological, orthopedic, and physical examinations.

3. Postural analysis

4. Functional assessment

Digital X-Ray Studies

Your specific condition may require us to order in-house films to get a full evaluation of your lifeline. Very specific x-rays allow us to pin point, measure, and determine the cause of you problems. When it comes to the importance, and crucial role of your spine and nervous system controlling your entire health, we will not guess on your health.  X-rays can also help us rule out more serious conditions or assist us in developing the most effective CAREplan for you.  Most people are amazed once they see their x-rays, and can often immediately identify their cause of the problem and degeneration.  Digital films will also help you see problems that you may not even know about (since symptoms are the last thing to show up with most health problems) and, most importantly, you'll learn how to correct them without drugs or surgery.

Our X-ray equipment is licensed in the State of Oregon and updated to provide the best quality images with the highest safety standards.

Personalized Report of Findings (ROF)

Once the consultations and TPC examinations have been performed, the Doctor will provide you an individualized Report of Findings (ROF) to answer the four most critical questions:

1. What's causing your condition.

2. How it's affecting your health right now.

3. What the initial requirements are for being accepted as a patient.

4. What the initial cost will be.

After discussing your history and your goals, TPC biostructural exam, and reviewing any x-rays or tests, the Doctor will discuss recommendations with you. For those reasons, it is a good idea to have your spouse or significant other at the ROF to be involved in your health decision.  If you have a condition that requires care more suitable for other providers, the Doctor will make the appropriate referral. If the Doctor believes chiropractic is a good fit for you, we will recommend a customized and appropriate CAREplan.

Corrective Chiropractic Care

Patients will generally be provided care at the conclusion of the ROF.  This may include your first spinal adjustment, active rehabilitation geared toward getting the most correction possible, and/or soft tissue therapies.  Treatment is interactive with you so you can express any concerns you have about the style of adjustment you will be most comfortable with. 

Corrective care can benefit the whole family!  Make sure to have your spouse and kids checked too.  Our philosophy is simple:
